Pseudoparaphimosis refers to the phenomenon that the foreskin covers the glans of the male penis when it is weak, but the glans can be naturally exposed when erect. Pseudoparaphimis may be caused by congenital developmental abnormalities, local inflammatory stimulation, obesity and other factors. It usually does not require special treatment, but daily cleaning and care need to be paid attention to.

    1. Congenital factors

Some men are born with delayed separation of the foreskin and glans during penis development, resulting in a larger foreskin coverage. This condition usually improves gradually with pubertal development and usually does not affect urination or sexual function. If there is still significant coverage in adulthood, progressive manual training can be considered to help the foreskin retract.

    2. Inflammation stimulation

Recurrent balanitis may lead to scarring stenosis of the foreskin orifice, resulting in secondary foreskin hyperplasia. Commonly seen in Candida albicans or bacterial infections, it manifests as local redness, swelling, and increased secretions. Compound ketoconazole cream, erythromycin ointment and other drugs need to be used to control the infection, and circumcision is performed if necessary.

    3. Effect of body fat

In obese patients, due to the accumulation of prepubic fat, the penis will appear visually short and the foreskin will be relatively long. This situation can be improved by losing weight. After losing weight, the degree of foreskin coverage will be reduced. It should be noted that excessive obesity may be complicated by diabetes and increase the risk of infection.

    4. Pathological adhesion

Physiological adhesions between the foreskin and glans are not completely separated in childhood and may continue into adulthood. The foreskin cannot turn up freely, which can easily hide dirt and cause infection. Foreskin expansion can be used to separate adhesions. Severe cases require surgical correction, and potassium permanganate solution can be used for postoperative care.

    5. Abnormal neural regulation

A small number of patients have abnormal sensitivity of the nerves that control the penis, resulting in imbalance of foreskin muscle tension. It may be accompanied by difficulty urinating or discomfort during sexual intercourse, and needs to be confirmed by urodynamic examination. For mild cases, biofeedback treatment can be tried, while for severe cases, foreskinplasty may be required to reconstruct function.

People with pseudoprephimosis should wash their foreskin every day to keep it clean and avoid using irritating lotions. Choose breathable cotton underwear and wash them before and after sex. If you have difficulty urinating, recurrent infections, or foreskin incarceration, you should see a urology department promptly. If pre-pubescent children are asymptomatic, they can be temporarily observed. Most will improve spontaneously with growth and development, and premature intervention is not recommended.
